Your clients are already waiting.
Mind Speak brings therapy to New Yorkers with intellectual and developmental disabilities, where they live. People are waiting for a therapist right now, and we're growing our network of licensed clinicians to reach them.
- 18 to 20
- clients from day one
- Monthly
- pay, before insurance pays us
- Yours
- the schedule, set with your clients

We handle everything but the therapy.
- 01
Clients ready for you, from day one
A caseload of 18 to 20 clients the day you finish orientation, with the referral details and the caseworker's name.
- 02
Everything but the therapy
Referrals, intake, insurance billing and the EHR are handled. You document; we bill.
- 03
Paid monthly, before insurance pays us
Invoice every Monday for the week before. Paid monthly for the prior 30 days, even though insurance takes six to eight weeks.
- 04
You set your schedule with your clients
Most sessions happen between 3 and 7 PM, after school, day programs or work. Weekends and early mornings if they suit you both.

A real week
You call your clients, their caseworker and a parent or guardian where that applies. You set days, times and places together. You go to them: family homes, group homes, their own apartments, around your borough or in Westchester. You log every session in the EHR and keep your notes within 24 hours. Nina reviews and approves notes before billing, and she coaches you, especially if IDD is new to you.
Never worked with people with intellectual disabilities? Good. Nina teaches. Some of our clinicians started that way.
Read this before you apply.
This isn't a 9 to 5, and it isn't for everyone. You set your schedule with your clients, you go to them, and you keep your notes and invoices on time. If that sounds like freedom, we should talk.
That sounds like freedomRight for
- A self-starter who finishes orientation and is on the phone scheduling clients the same day.
- Someone building their own practice, or who wants to, and treats Mind Speak like their best client.
- Someone who answers: returns calls, keeps the EHR current, sends the invoice Monday morning.
- Comfortable with people with intellectual disabilities, or open to learning. Nina teaches.
- Ready to work in the community: living rooms, group homes, stoops. Not an office.
Not for
- Someone looking for a salaried position with benefits, paid time off and set hours.
- Someone who wants clients to come to them, a fixed office or a fully remote caseload.
- Someone who struggles to keep documentation and invoices on time.
- Someone whose first answer is "that's not my job."

Meet Nina
Nina Ythier, LCSW, has spent nearly 30 years in the field, and City & State named her to its 2025 Trailblazers list. She interviews every clinician herself.
What clinicians ask
Is this a job or a contract?
A contract. You're an independent contractor (1099), building your practice with Mind Speak's clients. You set your schedule with your clients.
Which licenses do you accept?
LCSW, LMHC and LMFT, with an active New York license, current registration, and an NPI number. LMSWs: not right now, because Medicaid and Medicare stopped paying for LMSW services in October 2025. Leave your details and we'll reach out when you're licensed as an LCSW.
When do I get paid?
Monthly, for the prior 30 days of work. You send an invoice each Monday by 9 AM for the week before. Insurance takes six to eight weeks to pay us; we don't make you wait for it.
How many clients will I have?
18 to 20 to start, from day one. People are on a waitlist right now.
Who does the billing?
We do, through our billing company. You document in our EHR; Nina reviews and approves notes before claims go out.
I've never worked with people with intellectual disabilities. Is that a problem?
No, if you're open to learning. Nina coaches every new clinician, and she taught at the university level. Some of our clinicians had never done this work and now wouldn't do anything else.
Do I need malpractice insurance?
Yes, your own professional liability policy, on file before you see clients.
What happens after I apply?
You'll hear back quickly with next steps and a time to meet Nina. After the interview: an onboarding packet, a background check we pay for, and a one-hour orientation. Then you get your caseload and EHR login and start calling clients.
